Welcome to Calvary Christian Schools Athletics! We are excited to have your student participate in our athletic programs. To get started, all athletes must complete their registration through FinalForms. FinalForms allows you to easily complete, sign, and manage all required athletic forms electronically.


Next Steps:

    Visit FinalForms and create a parent account.
    

    2.

    Add your student and complete all required forms.
    

    3.

    Upload or verify a current physical (must meet OHSAA requirements).
    

    4.

    Select the sport(s) your student plans to participate in.
    

    5.

    Sign all forms electronically.
    

    6.
